Membership Terms

By enrolling in a LENA membership, you agree to the following terms:

Commitment & Billing

All memberships require a minimum commitment of three (3) consecutive months. Memberships are billed monthly on a recurring basis to the payment method on file.

Cancellations

Memberships may be canceled after the initial 3-month commitment with a minimum of 30 days’ written notice prior to your next billing cycle. Requests submitted after this window may result in one additional billing cycle.

Consultation Requirement

A consultation is required prior to receiving services to ensure treatments are appropriate and aligned with your care plan. LENA reserves the right to modify or decline services based on medical eligibility.

Membership Benefits

Membership benefits are non-transferable unless otherwise specified. Monthly services and perks are intended for use within the membership period unless otherwise stated (e.g., banked credits where applicable).

Banked Services & Credits

Eligible services may be carried forward as a credit for future use. Credits hold no cash value and may only be applied toward qualifying treatments.

Pricing & Modifications

LENA reserves the right to adjust membership pricing or benefits at any time. Members will be notified in advance of any changes affecting their plan.

General Terms

Memberships are non-refundable and may not be paused unless otherwise approved. Additional terms may apply based on specific membership tiers or promotional offerings.
